Leeds Senior Services Questionnaire

Good Afternoon,

My name is Alisa Burleson and I am the new Director of the Leeds Senior Services Department.  We are so excited to get things moving again in 2021 for our citizen's ages 50 and older.   As we develop new programs, events and services, input from our Senior Community is vital.  Attached is a survey (total of 3 pages) that will assist with this process.  Your help with the distribution of this survey would be most appreciated.   All surveys will be collected by the end of January and can either be turned into City Hall or you can contact me for scheduled pick-up.
